2026–2027 Athletic Participation Forms

All student-athletes must complete the required forms before participating in any UIL athletic program during the 2026–2027 school year.

Athletic packets are available from Coach Johnson or may be downloaded using the links provided below. In addition, all student-athletes must have an active RegisterMyAthlete.com account. Returning athletes are required to log in and update their information for the current school year before they are eligible to participate.
