2026–2027 Athletic Participation Forms

All student-athletes must complete the required forms before participating in any UIL athletic program during the 2026–2027 school year.

Athletic packets are available from Coach Williams & Coach Maddox or may be downloaded using the links provided below. In addition, all student-athletes must have an active RegisterMyAthlete.com account. Returning athletes are required to log in and update their information for the current school year before they are eligible to participate.
